体育运动 registration is done electronically through the ParentVue门户. 信息 regarding middle school athletics registration can be found 在这里. Below are some helpful resources to assist parents/guardians with the online registration process 和 with securing a physical evaluation prior to in-person engagement.

父母/guardians must complete the online registration process for students participating in the 威尼斯官网在线 interscholastic athletics program, prior to participating in practices or tryouts. As part of the online registration process for 威尼斯官网在线 athletics, students 和 parents/guardians are required to review health 和 safety information, including materials on baseline testing, 脑震荡, 心脏骤停.  Any questions about completing the online registration process should be directed to your local school 体育运动 Specialist or Athletic Coordinator. 

• Pre-participation物理 Evaluation Form - The Pre-participation物理 Evaluation Form is valid for 13 months (for high school athletes) 和 two years (for middle school athletes) from the date of the exam, 和 covers participation for all sports within that time period. 父母/guardians submit the last page to the school/coach (the Clearance Form). 学校 keep the forms on file.  部分列表 Clinics that offer Sports Physicals within the Montgomery County area is available online.

This form must be submitted for each sport in which the student wishes to participate.

• Consent Form ImPact Baseline Concussion Testing - All student-athletes are encourage to complete a baseline test prior to their initial participation in the 威尼斯官网在线 athletics program. The local school provides access to baseline testing, please reach out to the local school athletics specialist for more information. This form is required for taking a baseline test.
• MSDE Concussion Awareness 和 心脏性猝死 Arrest Awareness 家长/学生-Athlete Acknowledgement Statement -  This form indicates that the parent 和 student-athlete have received information regarding 脑震荡 心脏骤停.
• MSDE Pre-participation Head Injury/Concussion Reporting Form for Extracurricular Activities - This form indicates whether the student has previously suffered a concussion.
